Child Death Overview Panel
Purpose
The Child Death Overview Panel will:

Develop systems to enable the collation and analysis of information about the deaths of all children in Lincolnshire with a view to identifying:

Any matters of concern affecting the welfare and safety of children including any case giving rise to the need for a Serious Case Review. Serious Case Reviews are officially authorised by LSCB or by the Chair of the LSCB on behalf of the LSCB. “Working Together to Safeguard Children” explains the Group’s Terms of Reference and illustrates the criteria for convening a Serious Case Review

Any matters of concern affecting the safety and welfare of children in the area of the authority

Any wider public health or safety concerns arising from a particular death or from a pattern of deaths in that area.
The Panel will also put in place procedures for ensuring that there is a coordinated response by the authority, their Board partners and other relevant persons to an unexpected death of a child.
